I reccomend going by bus or train to Przemysl, from there by small bus to Medyka (they go very often), there is pedestrian passage throguht the border (and the passage is much faster than car/bus/train one).
So go on foot throught the passage (some formalities there - you need to sign a paper which says where you will station in your travel destination in Ukraine - you can just write any address in Lvov, they don't check that, but you have to keep the paper when you, Ukrainian officers will check if you have the paper when you'll be returning to Poland).
If you go by train/bus/car - toll officers will check much more thoroughly if you and other people passing border don't have anything illegal, and it's much longer queue (and it's going much slower), so if you don't want to wait on the border too much - I reccomend going the pedestrian way). Besides - Przemyśl city is quite nice, you can visit it on your way to Lviv.
On the other side of the border near Medyka (it's called Shegini or sth like that in Ukrainian) there are small private busses directly to Lviv, and they were quite cheap. They are going out often (more often than once a hour IIRC.
I was visiting Lviv that way in 2008 and the travel was nice and easy, much less hassle than going by train.
