I suggest you start by contacting the official Kaunas Tourist Information Office:

info@kaunastic.lt

They should be able to put you in contact with local guides.

I have found contact details (although no email) for one Hebrew-speaking guide who lives in Kaunas and for several others who live in Vilnius. I have no knowledge of them, whether they are still available or their charges. You can find their contact details in English here:

The pusalotas page also has contact info for for the Jewish Community Centre in Kaunas. It might be worth contacting them as well.

I think it would be worth contacting the Hebrew-speaking guide who lives in Kaunas listed on the pusalotas page. He has no email, unfortunately, but perhaps you could write a letter to him (or phone him, if you are happy with the probable charges)?
