Public transport in Singapore is outstanding. You'll get to most places on the MRT, bus or taxi.

Visit:

Chinatown (see Chinatown Heritage Centre; Sri Mariamman Temple; Buddha Tooth Relic Temple; eat at Maxwell Road and Chinatown Complex Food Centres as well as Night Market).

Orchard Road (many shopping centres like Tangs, Wisma Atria, Ngee Ann City, Paragon, The Heeren, The Centrepoint).

Jurong Bird Park.

Singapore River (Clarke & Boat Quay, Merlion, Esplanade Theatres on the Bay)

Little India (Sri Veeramakaliamman Temle, Mustafa Centre, Sri Srinivasa Perumal Temple).

Kampong Glam (Bugis Junction, Haji Lane, Bussorah Street, Sultan Mosque, Malay Heritage Centre, Istana Compound).

Singapore Zoo.

Colonial Singapore (Padang, Supreme Court, Raffles Landing Site, Asian Civilisations Museum, Cavenagh Bridge).

Singapore Botanic Gardens.

Geylang (Sri Sivan Temple, Foo Hai Chan Monastery).

Enjoy!

Always check for yourself but we did not take any malaria preventive nor did our travel doctor advise us to do so.

I skimmed the other VT answer but did not notice if trishaws were mentioned.

We had a "hair raising ' ride on one from Raffles back to Orchard Road.Probably peak hour not the best time.

Johan already listed most of the attractions in SG. Malaria pills are not necessary here. Transportation is very punctual and user friendly. If you will stay more than a few days in SG, get an easy link card to travel with MRT and bus services. You an find many things to do and restaurant tips in my Singapore page.

For Malaysia, you have many options. You can travel by bus or by plane to many destinations from SG. If you give more details about your duration of stay, we can give better suggestions. Melaka and KL are 2 popular cities to visit. Langkawi is an island you don't want to miss. You can check my pages for more details.

Enjoy SG and Malaysia :)

The bus station located at Golden Mile Complex, Beach Road, Singapore provides express bus services to all major cities in Malaysia, even Thailand, if you like train travel, go to "seat61.com" for more info.

Relatively safe in Singapore. No you dont need malaria pills unless you are really going into foresty places which are quite rare in Singapore

Since Singapore is small, you can tour it in 2 days or so. Here is my detailed itinerary on what to see http://www.ehow.com/how_4906680_stay-organized-singapore.html

whatever you do, don't miss the Night Safari of Singapore.

Kuala Lumpur, Malaysia is possible via an overnight train from Singapore. There are a lot of places to explore in Malaysia.
