Well if VT does not know a travel doctor would. Also I think you can find out on the internet if you search. Also where you obtain passports and visas or a travel agent are other options.
You have to visit a travel doctor anyway to obtain the shots and the certificate.

This is information from the Centres for Disease Control and Prevention (http://wwwn.cdc.gov/travel/yellowbook/ch5/malaria-yellow-fever-table.aspx):

"Required for all travelers >1 year of age. Medical waivers must be translated into Spanish and accompany the International Certificate of Vaccination or Prophylaxis (ICVP). Travelers who do not have a valid ICVP will still be allowed to enter Bolivia if they agree to sign an affidavit exempting the Bolivian state from any liability in the event the traveler gets sick with yellow fever within the Bolivian territory. This last option may cause delays at the point of entry."

However, when I travelled from Peru to Bolivia nothing was required at all, and I did not sign anything. But sure, it could have changed since I was there some years ago. I would surely not get a vaccination if I do not travel in the affected lowlands, so would not have a certificate. So I would just sign this affidavit at the cost of a little delay at the border.
