I was just looking up the same question for someone going to Gardone del Garda (partway up the lake on the west side), and noticed that the buses tend to leave Desenzano del Garda about 30 minutes after each connection from Milan stops there.
Yep, look at http://www.apt.vr.it/pdf/80.pdf - it's the same bus, so look at www.trenitalia.com for rail solutions to Desenzano del Garda and compare with the bus schedule. Note that the trip is almost 2 hours, and that not all buses on this line go all the way to Riva.
Now, I will note that there appears to be a ferry that runs all the way from Desenzano del Garda to Riva del Garda (the entire length of the lake), but I know nothing about it; perhaps someone will join in.
The Malpensa website does not report any direct buses to the immediate area, and this doesn't surprise me. You can take a bus to Brescia or Verona, but then you're no better off than if you had taken the train. (unfortunately, the bus to Verona goes past Desenzano del Garda, but doesn't offer a chance to stop there)
