This is a tough one...
Arrival in Malpensa airport, Milan at 12/1/09 22:15.
Could not rent a car, so at that late hour I have to find a way to go to the Hotel Turismo (Via Nazionale 58, SAN MARTINO BUON ALB 37036)that is a bit outside of Verona.
I could only find a train to Verona, which has a 4h stop in Milan, Centre.
Some good ideas would really help..
ps. maybe someone knows a rental company that accepts MAETRO or warrant in cash?

I looked at www.ferroviedellostato.it
and Malpensa web site

and it says that you can use a train from GALLARATE station (not the Milan City train) and that this station is reached by a bus.
According to Trenitalia there may be a train from Gallarate that goes around Milan (not going into the centre) and you can change at Pioltello-Limito (east of Milan) for the Verona train. I think you could look at this option? It appears that a train leaves GALLARATE at 23.09 and reaches Piotello at 00.20 then a new train leaves Piotella at 00.30 and gets to Verona at 02.23

The train that Nige refers to actually starts at Milano Centrale (regional #10601) at 00:15.
However, this leaves you with the problem of getting to Milano Centrale. One way (sort of) is, as he suggests, taking the FS train from Gallarate to Pioltello to meet the regional train from Milano Centrale as it goes to Verona. However, this has 2 issues: the train leaves Gallarate at 23:09, so you have to be sure that you can get your bags and through immigration (as needed) and get over to the train station in Gallarate in less than 1 hour...yes, there are buses, but I would take the taxi just to be sure (it's 15 minutes or less). The other issue is that the connection time in Pioltello is just 10 minutes, so if the train from Gallarate to Piotello is late for whatever reason, then you would be stuck in the middle of nowhere in the middle of the night, with the next train not for hours.
Alternatively, you can take the bus from Malpensa down to Milano Centrale to catch this same train (#10601) originating there, departing there at 00:15. This presents similar problems...since the bus ride down to Milano Centrale is about an hour (although traffic should not be an issue at that time of night), and there is up to a 20-30 minute wait between buses (see the sea-aeroportimilano.it website that Nige referred to), again, you may have difficulty making the trip in a timely fashion. Could you? Yes, but it could be very close, depending on how much time it takes you to get out of the airport. And a taxi from Malpensa to Milano Centrale costs 70 euro(!!).
I suppose you have already looked for a flight to Verona from Milan? I guess it's late, but that would be faster, since you are already at the airport.
Your problem is that you are arriving fairly late at an airport that is about 200 kilometers away from your destination...and Italian public transit starts slowing down after 11 p.m. and nearly stops after midnight.

Oh, and if you are able to take #10601 and get into Verona at 02:23, I hope there is a taxi at the station(!)...in the larger cities, there will be, but I am not sure about Verona ;-) maybe my friend Francesco (VT: effeti) knows...
This hotel is 7-8 kilometers from the train station, and if there are any buses at all running at that hours, they would be very few (maybe one every hour or hour and a half) - don't count on it.
P.S. if you spend the night in Milan, you can take several trains from Milano Centrale to S. Martino Buonalbergo - the train station appears to be just 1 kilometer from the hotel (and surely there are buses and taxis during the day)...but the 10601 train does not appear to stop there...

I noticed a gueue of taxis at verona station when we arrived in the afternoon
btw there are two stations in verona - i can't remember which is the central one

Verona Porta Nuova is the central (main) one...yeah, my concern was taxis at the station at 2:30 in the morning...;-)

Once arrived at Verona Porta Nuova in the deep of night, no extra-urban buses at all.
So, the only choice is a taxi.
I never had to catch a taxi at those time, but i an pretty sure a few will be working.
They claim the service is 24 hours a day.

Now I am really stressed. I think getting the bus - rushing for the train - calling a radiotaxi stands greater chance!

Uh, please note that San Martino Buon Albergo (St. Martin "good hotel"), though not so far, is out of the city limits of Verona. So you will have double extras... 1) out of town extra 2) night extra.
Can you simply swap that night with one of the cheap hotels close to the train stations of Verona? (Piccolo and novo hotel rossi come to my mind...).
The following day you will have a cheap bus to San Martino.
