How about Lago Maggiore? It's an easy day trip from Milan.

Why don't you look at people's travel pages for Milan and see what they have suggested for day trips? You are spoiled for choice. The direct train from Milan to Naples stops at Bologna (1h42), Florence (2h44) and Rome. The one to Venice (3h) stops at Brescia, Desenzano del Garda, Peschiera del Garda, Verona, Vicenza and Padua. Pavia is only half an hour away, on the line going to Genova, S. Margherita, Monterosso, Pisa, Livorno (and in the other direction to Monza, Como S. Giovanni, Lugano, Bellinzona ...).
