No passport control I have ever passed through, including the Czech Republic, has ever asked for anything other than my passport/identity card.
However, I am an EU citizen and the only non-EU country I have visited so far is Turkey. You do not state your citizenship; matters may vary from country to country for non-EU citizens. If you are a UK citizen, one applies for and carries the European Health Insurance Card to obtain free health care in EU countries (and a counple of others). It replaced the old E111 form. Details here: http://www.dh.gov.uk/en/Healthcare/Healthadvicefor You would, however, be foolish to travel anywhere without the appropriate health insurance, even within the EU as an EU citizen. Whilst, in the latter case, reciprocal agreements mean treatment will largely be either free or (eventually)refunded, the cost of transport home can be massive. There is no reimbursement for that.

As said before, since you do not tell us your nationality, it is impossible to give a full answer. General rule: If you need a visa to go to Schengen, you also need proof of insurance.

You did not say where are you from; there are no passport controls on the Czech borders anymore. Controls at the airport dont check the insurance, however I think it is not too wise to have any insurance while traveling abroad.

You need proof of insurance in order to get a Schengen visa, I believe.
Therefore there is no need to check at aiports etc: the existence of the Schengen visa is proof that insurance is carried.
