Does anyone know the best way to get to Espinosa? Fly into Belo Horizonte and drive, or bus, or ? Or Fly into Brasilia and car or bus?

I'd say fly into Salvador and go by bus. Brasilia is also a good option (smack in the middle). BH is much farther away.

If you have the permission to drive in Brazil go by car/van.
You may head to Espinosa from Belo Horizonte/MG, or, Salvador/BA. Espinosa is very near border Minas Gerais/Bahia. The paved route road from Belo Horizonte is: Belo Horizonte-->Curvelo-->Montes Claros (This move made on
BR-135 paved road). The other move is Montes Claros--> Capitão Eneas--> Janauba--> Porteirinha--> Monte Azul--> Espinosa (This move made on BR-122 paved road).
If you want to soften your trip you may use an air-taxi from Belo Horizonte to Monte Azul regional airport. Monte Azul is the nearest city to Espinosa with regional airport, and is 23 Km far from your final destination.

Fly into Belo Horizonte. Take the bus to Montes Claros and then the local bus to Espinosa.

Be prepared to be a long time on the road. A U.S. east coast city to B.H. can be about 19 hours of travel. Overnight in B.H. and another long day on the bus. (I would not advise the usually good idea of leito/semi-leito overnight sleeper buses on this route.)

btw, If you are visiting someone (and why else would you be going to the middle of nowhere?), they should be helping you get there.
Air taxi/small regional air line will be ruinously expensive.

(Also, advising someone to drive when you are not certain s/he is used to driving and drivers in Brazil, and may not speak Portuguese, particularly with road conditions in that area, IMO, is totally IRRESPONSIBLE.)

I agree wholeheartedly with baiana77. The roads in Minas Gerais (especially north of BH) are some of the worst and most dangerous in Brazil. Definately not a good idea to drive.
