I've heard $100 notes were not accepted, even by banks for a while. Is this still the case?

You will have trouble with this denomination in small towns, but most major banks in major cities like Quito will honor US $100. Good luck!

I wouldn't carry anything larger than $20s. Ideally you have lots of $1s and $5s because making change in Ecuador can be difficult.
There are ATMs in most cities so carrying large amounts of cash isn't really necessary.
