During previous trips to Maui, I always purchased the card ahead of time and in the past a printed page would have a bar code that would work when arriving on the islands. Now I think they actually send a real card.

That's why purchasing online worked for me as I printed off the email with the bar code.
I don't know if they still do it that way. Regardless, when you land in Kahalui or Honolulu or Hilo, look for the "This week in ..." tourist pamphlets are there are some great savings coupons in there.
