Is there an airline that is a direct flight from Molokai to kaui? Also, is it necessary to rent a jeep instead of a car on Molokai?

I do not believe so on the first question and NO on the second.
Island Air connects through Kahului, Maui. Mokulele Air connects through Honolulu. Even then these smaller carriers have very limited service times per day. The route from Ho'olehua to Lihue is just not one that is in high demand. Would a charter suffice?

There's no direct flight but it's now easier to get there with Go! Airlines. Check out their schedule. For example, you can leave Kauai at 8:15am and arrive Molokai at 10:07am with a connection in Honolulu. We were on Molokai last year and a car worked fine. If you ever go to Lanai, however, you'll need a jeep!
