Hi, You certainly need a special permit for Tibet. You can try and find a travelagency in Beijing which gets you the trainticket and the permit. It is very difficult to get the reservations for the train. The tickets can be bought earliest 10 days before departure. Normally the Tibet-permit is only issued to groups of at least 5 persons. But Chinese travelagencies may know other possibilities.

AFAIK, the situation changed recently because it was so much abuse and it's much easier now. Some locals will certainly show up and tell in detail soon.
Don't forget to have a check-up with the doctor anyway before you go, 5000 m is not a joke.

Yes, you need the standard TTB permit. There're of course some travellers who've bought their own tickets and gotten away with not having a permit, but it's hit-and-miss.

It's difficult to answer your question because the Chinese government kept changing their rules. But in the past one year, most of the travelers could just come BY TRAIN without any problem. You can buy the train ticket in Beijing by yourself, officially you cannot, but practically you surely can. There's even a VIP booth located on the 4th floor of the Beijing Central Station and they will sell you tickets. If you are refused, just keep asking.
On the train, they won't check your permit.

I am sorry, but I would not rely on this answer. Local people in China normally do not have an idea about the difficulties, Western tourists may have travelling in China and specially in Tibet. In some cases it may have been, that people could just hop on the train, But I know people, who did not succeed with that. It may be a case of how much time you have. If you have time enough to wait until you get a ticket, or if you have time enough to spend days in a Tibetian police station, than try it.
Good luck!
