you'll have to get a bus to la avellana which their should be direct about every hour from somewhere in guate but i don't know where. from there you have a 20 minute boat ride to monterrico which departs when it fills.
alternatively you can take a more frequent bus to esquintla and then a bus to monterrico.

From guatemala city you would have to take a chicken bus to Puerto de San Jose and from there take a launch to get to Monterrico. The problem is that the areas where you would have to catch the bus are not very safe. So, why not take shuttle van to Antigua from the airport, spend the night there and take a shuttle van to Monterrico from there. I think safety has to come first. Or write to one of this hotels and ask them for advice www.cafe-del-sol.com or www.hotelateliedelmar.com.
