When are you going there, what do you want to visit and what are we talking about: wheelchair, walking stick? I ask because as far as I remember, Santo Domingo streets aren't very handicap friendly (no offense) but since the city is constantly changing that might have changed.
I'm going in 2,5 weeks and I can find out for you.

Thanks is ever so kind of you. I'm ambulant and only require a wheelchair on occasions.

I'm very sad to say that most of Santo Domingo isn't yet wheelchair friendly, but the modern areas are less harsh: there are pedestrian traffic lights at the big intersections (some with a timer that counts down how long before changing the light), the modern buildings have wide elevators and bathrooms but as far as ramps that's basically nonexistent but people will help you if you ask them nicely. If you go to the Old Town have someone push your wheelchair.
If you need a walking stick as aid then you're more or less set up there. And of course, if you need more time to cross the street people will help you out.
