If necessary, where is the best place to get the permit done. I understand there're many touts and scams upon visa. Please enlighten me. Thanks!

Yes, you still need a permit. A good place to get on is in Chengdu

You need a visa / permit for the Tibet autonomous region to go to Lhasa. How easy it is to get it depends on where you are travelling from. If you are going to Tibet from Nepal then it is best to get it done in Nepal as it is cheap and you don't even need to go to the Chinese Embassy (the travel agent does it for you). The cost is US$45 for the visa which is not stamped on your passport but given as a paper visa. They will only stamp your passport on entry/exit. Hope this helps.

Cheers,
Aadil.

Chengdu is a great place to get the permit, from other Chinese cities usually the travel agencies can arrange for them.
I am still slightly confused re the permit thing, as when we went to Lhasa in September this year I was told in fact there is no premit required for entering and staying Lhasa. Just going out of Lhasa to the back countries and other towns in Tibet requires travel permit.

Yes, you still need a separate TTB permit to go to Lhasa.
Chengdu, Xining and Beijing are probably the best gateways.
