Any advice please on getting to Pulau Perhentian (either Besar or Kecil) from Kuala Lumpur please?

Fly AirAsia or Malaysian Airlines to Kota Bharu. Then take a taxi to Kuala Besut Jetty. Catch a boat to Perhentian from there. Most ferries/boat leaves in the mornings or late mornings. So catch the earliest flights.
Cheaper option. Take a overnight coach from KL to Kota Bharu. Upon reaching Kota Bharu Bus Station, follow the same procedure. Take taxi to Kuala Besut.......

I've booked flights, but won't be getting to Kota Bharu until 4.35pm... so don't expect to get to the Kuala Besut jetty until about 6pm. Is this too late to get a ferry??

Definately too late for the ferry unless of course you willing to charter a private boat. It will be pricey though. The option is to stay overnight in Kota Bharu or Besut to catch the next morning ferry.
Have you pre-booked any resort in Perhentian?? They might be able to help you out or advice you on the options available to you.
