Do I need bug repellant in any of SV, Cisco, Puno< Lake Titicaca in March?

I would recommend you to get some mosquito repellent if you go to Machu Picchu, especially if you do the Inca Trail. Machu Picchu is in the limit with the high jungle area, so mosquitoes could be an issue depending on the humidity and weather conditions. When I went there in July 1998, I got various bits on the arms.
Besides, what Pickledonions says is right: the higher the place the less bugs you will find. Therefore you won't need the repellent in SV, Cusco (except MP) and Puno.

Its good practice to have a small insect repelent with DEET. But as Pickledonions said, high elevation like Cusco, Puno will keep the biters off, but there are crawly things and itchy things if you go romping around Lake T.

In Machu Picchu - YES!!!! When I was there in October we were bitten to bits and that was even using 50% DEET. Even in the Sacred Valley you get some mozzies. Be prepared!! As you go higher, you don't get bitten - Cuzco and above.
