Very easy. There are dozens of travel agencies in Lhasa that can arrange it for you when you arrive. There is no need to arrange it beforehand.

4WD's from Lhasa can't take you all the way to KTM. They can only take you to the border. From there you have to cross the bridge into Nepal and arrange your own ride into KTM.
The cost is going to vary. Do you want to get there in one day and not see anything? Do you want to take the standard 5 to 7 day trip that all the other foreigners take or do you want to do something else? With travel agencies in Lhasa, you decide where you want to go and how long you want to take. These are not set trips. If you want to take 6 months, then you can do that. If you want to take a week, you can do that as well. There are local Tibetans who make the trip to the border several times a week. They park their 4WD's in front of the Gang Gyan Hotel on Beijing Road in Lhasa. You should be able to arrange transport from these guys for around Y800 to Y1000 per person. They leave in the morning and get to the border in early evening. They will not stop at any place along the way except for fuel. If you opt for what all the other foreigners do...the 5 to 7 day trip via Yamdrok Lake, Gyantse, Shigatse, Sakya and Everest...it will cost around Y4200 per vehicle. This price includes all permits, but doenst include your food or accomodations. Be sure to negotiate hard on prices. You are going in the low season so you should get a much better price than the summer time.
