You can fly or take the train into Rome. If you fly, take the train to Termini station downtown Rome. From there you can get a bus to the Vatican (or walk, as we did).
You enter St. Peter's Basilique free. It is a church and open to the public. To visit the Vatican Museum, you will have to pay. I don't remember the price but I'm sure someone here will have that information. As far as Vatican City as a city, you really can't tell when you leave Rome and enter Vatican City. You just walk down the street and you are there. There is no charge; there is no passport check. They do have their own post office and it's fun to send post cards from it. When you stand facing St. Peter's, the Vatican Post Office is on your left.

If you arrive at Fiumicino airport, you can go by train to Roma Termini station (+/- 1/2 hr, 8.80 EUR) or by Metropolitan trainline FM1 to Roma Tiburtina station (+/- 3/4 hr 2.27 EUR); as the metro station closest to the Vatican is OTTAVIANO on metro line A (or the next stop, Cipro, for the Vatican museums), TERMINI would be the better option as metro lines A and B both pass through here whereas Tiburtina is on line B, so you would have to go from there to Termini to change to line A. Alternatively there are shuttle buses from Fiumicino to Magliana (Line B) or Lepanto (Line A) metro stations (30-40 min, 3 EUR), here obviously LEPANTO would be the better choice if you are going directly to the Vatican, as it is only one stop from Ottaviano.
Entrance to the Vatican museums costs 12 EUR, opening hours April-Oct are Mon-Fri 08.45-15.45, Sat 08.45-13.45 (last admission 14.20), free on the last Sunday of the month, same hours as Sat, closed Catholic holidays. In high season it is best to go on a weekday afternoon, Saturdays are unbearably busy. Reservations are not required, all information you require is on here http://mv.vatican.va/3_EN/pages/z-Info/MV_Info_Ora
