There are many choices when it comes to renting cars in El Salvador. All of the US companies such as Avis, Hertz, National, and Budget have agencies inside or near the international airport. Prices depend on what type of car you want to rent but they start at around $25/day. Although, there might be additional fees if you rent at the airport since there is an additional airport concession fee and a higher tax they add on to the rental.
I don't know if it has changed, but last time I rented at the airport I was not able to decline the collision coverage even though my credit card would have covered me for any damages. Since then, I have preferred to rent used cards from local agencies in the capital city. Depending on the agency and the length of the rental, you can get discounts on the daily rate and are able to turn down collision coverage if you use a credit card that will cover you. I was there in March and rented from Sandoval & Cia. (www.sandovalrentacar.com). There are other agencies on the net as well such as www.eurorent.com.sv and many others by doing a serch online. I don't know if the ones I have listed have any agencies out in San Miguel but if you are staying in San Salvador at all, you can rent there and take the car out to San Miguel. Otherwise, I'm sure there are plenty of car rental agencies out in San Miguel - just look in the local yellow pages once you get there.
As far as hotels, there are plenty, it just depends the level of comfort you want. Ranges are from those that offer just the basics (no a/c - something I don't recommend, especially out in San Miguel) to more US type acccomodations. Comfort Inn recently opened up shop - I believe rates start at around $70/night for a double room. Just do a search for the city of San Miguel in El Salvador on www.choicehotels.com to make reservations online and look at pictures of the accomodations. Have fun on your trip.
