We will be staying in Sandakan at the Sabah Hotel at the end of April. Is there a danger of malaria and should we be taking a malaria prophylaxis? We have determined that we do not need it anywhere else we will be visiting. We will be on a tour. The group does not advise about malaria.

The Centers for Disease Control and Prevention (CDC) have a lot of great information about Malaria and other travel bugs on their website. I have given the link below.

general answer - no. If you intend to stay in the forest then yes, but in tours you're fine. even visiting the Sepilok sanctuary is fine, so Iwouldn't worry about it at all. For peace of mind though, it's your call.

In a word YES, Sabah is a malaria area and antimalarials should be taken, see your doctor for advice and accept no other advice on standard antimalarials except from a qualified source, we used a homeopathic one (demal 200 from gift of africa) in Sabah but if you go with that one esure you consult your doctor before using it. If you are visiting the forest, for example near the Kinabatangan river or where there is water, yes it is better to be safe than sorry and take the tablets. Always speak to a GP or your immunization clinic for the best advice. For Borneo immunization clinics always recommend malaria tablets.

I suppose it is too late for you, but for others seeking informarion, be aware of Dengue Fever too. It is also spread by mosquitos.
