What is the best way to get to Genoa, Italy from the Milan Malpensa Airport? Exact directions would be helpful. I've never been to europe before. is there much english spoken or posted on signs etc?

From Malpensa (Gallarate) by highway A8 in direction of Milano, do not enter in the city, take TANGENZIALE OVEST in direction of Bologna, after passing Trezzano sul Naviglio and Buccinasco turn to A7 for Genova.

So give us a hint - how to you plan to get to Genoa? Train? Airplane? Hitchhike? It's difficult to answer your question otherwise.

Also, please define what is "best" for you? Cheapest? Fastest? Most scenic?

Yes, people speak some English in Italy, but it varies. Airline employees mostly speak English, while drivers picking you up hitchhiking may not speak any.
