It is your citizenship which matters in terms of visas for any country. If you are a UK citizen (I think you are?) then you need to either get an ESTA before travel or a visa. This is the official site for getting an ESTA: https://esta.cbp.dhs.gov/esta/ Don't use another site and don't pay any more than the official fee (7USD I think, but I got my ESTA before charges came in). FAQs here: esta.cbp.dhs.gov/esta/WebHel... In the unlikely event of you finding you are not eligible for an ESTA (assuming you are a UK citizen) then I'd advise you to make your visa appointment as soon as possible. Official site: london.usembassy.gov/apply.h...

By the way, sites which claim 'cheapest fees' actually mean 'cheapest fees for sites which charge more than the official US embassy site does'! Use the official site. :-)

This is the official site: https://esta.cbp.dhs.gov/esta/ I would use that one as the other ones might charge you an extra fee for the service and here you deal directly with the people in charge. I have used it and it was very easy and fast to get.

Wherever you plan to travel it's always a good idea to first get your visa info from the relevant official embassy website. There are lots of agencies and websites out there who will 'help'....but always for an extra fee.

You'll almost certainly get your UK>US ESTA immediately (you can print it out yourself but don't actually need to show it: the info is passed electronically so 'they' know you have an ESTA when you enter the US) But it's best not to leave it until the last minute in case, for some reason, you find it is not applicable for you or your application is refused. I'd apply now for a September trip...an ESTA lasts for 2 years and is multiple-entry. I'll shut up now....:-)

As previous posters have pointed out, if you are a British citizen with an e-passport travelling to the USA for a brief visit, you don't need a visa but must apply on-line for ESTA authorisation. If you have a passport which is not machine-readable (unlikely, unless your passport is nearing expiry or was issued years ago at a British diplomatic mission) or if you are travelling to the USA for work/study, you will need a visa.

Totally agree with what is written we nearly got caught with agencies etc. We paid14 through the ESTA website all done electronically and my wife didn't have to show it on arrival

For the above posts did not directly answer your question about the I-94W Form. This form is your arrival declaration form, typically handed out in the plane prior to arrival. If not, they will have them in the immigration area desks.
