Firstly I would say that Karachi to Naran is a long way and would require proper planning to make it fun. If you and your family have prior traveling experience than it would be great help. If no than you have to spend some good time on the planning aspect for which you have just good enough time imo.

Usually people hire Hiace from Islamabad as Karachi-Islamabad can be done through more reliable means. But if you have locked into it then no problems, otherwise, I may suggest you the alternatives.

so here is the itinerary you may like to consider:

Day 1: Depart from Karachi early in the morning. Stay overnight in Bahalpur
Day 2: Bahalpur to Rawalpindi/Islamabad. Night stay in Rawalpindi/Islamabad.
Day 3: Rawalpindi/Islamabad to Shogran. Night stay in Shogran.
Day 4: Half a day trip to Shogran (requires jeep hire). Night stay in Shogran
Day 5: Shogran to Naran. Night stay in Naran
Day 6: Day trip to Lake Saiful Mulook (requires jeep hire). Night stay in Naran.
Day 7: Roam around Naran. Night stay in Naran.
Day 7 (alternative): Naran - Balakot. Night stay in Balakot.
Day 8: Naran/Balakot to Nathiagali. Night stay in Nathiagali
Day 9: Roam around galiyat. Night stay in Nathiagali
Day 10: Nathigali to Islamabad. Enorute Murree. Nights stay in Islamabad.
Day 11: Explore Islamabad/Rawalpindi. Day trip to Taxila is an option.
Day 12: Islamabad - Bahalpur. see Bahalpur zoo. night stay
Day 13: Bahawalpur - Karachi. leave early morning

So these are some clues for your to start the planning. Feel free to come back id you need further assistance.

Note that bargaining is the key word when it comes to selecting a hotel. Prices vary a lot depending on how much one can pay!

Another important thing, do not spend less than one night at one place (except for while traveling Karachi-Islamabad and back) so that you don't waste a lot of time in packing/unpacking...

    Day 12:

Muzamil has given a good itinerary.

In-route there are couple of other places that you could see:

    1/ Near Sukkur/Larkana Moen-jo-Daro.
2/ Before Bhawalpur, close to Ahemdabad, there is Uch Sharif and Delawar Fort
3/ In-route between Multan and Lahore before Sahiwal there is Harappa.
4/ Lahore: visit the varius forts and gardens
5/ on motorway near Pind Dadan Khan Khewra Salt Mines
6/ Naran: Do visit Lulusar Lake, Babusar Top and Nuri top could be done in a day if you leave early.

Best place to stay is the PTDC motel, but there are more hotels there now. It will be peak season so make sure that you book in advance.

    Thanks Amir bhai..

you have given some great ideas too. this makes good sense especially if one is taking their own transport from Karachi.

@ OP: when parents used to take us to Pakistan trips, my mom had to always keep a small gas stove which was very helpful for tea etc... just an idea for you too not only because it saves a lot of money but more because you can get your own 'homely' tea on the road!

Well dont need stoves anymore :) Everything now is available
In-route best place to stop for rest/restroom break, tuc shop to purchase water, tea etc are the CNG stations. They are much cleaner comparred to petrol stations.
