Do we need to make arrangements in advance? How can we find an english speaking driver? What will it cost each way? Can we design our own itinerary to Taormina?
Mesina to Taormina is 50+km so a taxi will not be cheap. Is it essential that you have a taxi? A taxi may possibly let you design the itinerary as well, but will certainly charge you for doing so. There are regular Messina>Taormina buses. Timetable here: http://www.etnatrasporti.it/

Don't know what it would cost you to have a driver and vehicle in today's economic market, but it doesn't hurt to ask the source.
